BACKGROUND: Catatonia is a challenging and heterogeneous neuropsychiatric syndrome of motor, affective and behavioral dysregulation which has been associated with multiple disorders such as structural brain lesions, systemic diseases, and psychiatric disorders. This systematic review summarized and compared functional neuroimaging abnormalities in catatonia associated with psychiatric and medical conditions. METHODS: Using PRISMA methods, we completed a systematic review of 6 databases from inception to February 7th, 2024 of patients with catatonia that had functional neuroimaging performed. RESULTS: A total of 309 studies were identified through the systematic search and 62 met the criteria for full-text review. A total of 15 studies reported patients with catatonia associated with a psychiatric disorder (n = 241) and one study reported catatonia associated with another medical condition, involving patients with N-methyl-d-aspartate receptor antibody encephalitis (n = 23). Findings varied across disorders, with hyperactivity observed in areas like the prefrontal cortex (PFC), the supplementary motor area (SMA) and the ventral pre-motor cortex in acute catatonia associated to a psychiatric disorder, hypoactivity in PFC, the parietal cortex, and the SMA in catatonia associated to a medical condition, and mixed metabolic activity in the study on catatonia linked to a medical condition. CONCLUSION: Findings support the theory of dysfunction in cortico-striatal-thalamic, cortico-cerebellar, anterior cingulate-medial orbitofrontal, and lateral orbitofrontal networks in catatonia. However, the majority of the literature focuses on schizophrenia spectrum disorders, leaving the pathophysiologic characteristics of catatonia in other disorders less understood. This review highlights the need for further research to elucidate the pathophysiology of catatonia across various disorders.

Introduction: Millions of snake bites occur worldwide each year. Clinical practice guidelines generally do not recommend the use of prophylactic antibiotics. Objective: To determine the sociodemographic, clinical, and pharmacological variables and the use of antibiotics in a group of patients with snake bites in Colombia. Methods: A retrospective cross-sectional study was carried out. Patients affiliated with a Colombian health insurer who presented with snake bites between 2015 and 2022 were included. The cases were identified from the National Public Health Surveillance System. Sociodemographic, clinical and pharmacological variables were identified. Descriptive and bivariate analyses were performed. Results: A total of 643 patients were analyzed, with a median age of 30.8 years, and 74.7% were men. The most frequently identified genus of snake was Bothrops (88.8%), and most incidents were classified as mild ophidian accidents (61.6%). A total of 59.7% of patients received snake antivenom. A total of 13.8% and 2.2% of the patients had cellulitis or abscesses, respectively. A total of 63.5% received antibiotics (50.6% for prophylaxis and 12.9% for treatment), especially cephalexin (25.9%), and most of the antibiotic management was considered inappropriate (91.7%). Conclusion: Most patients with snake bites received antibiotics, especially for prophylactic purposes, a clinical behavior that goes against current evidence. The use of antibiotics with an unsuitable spectrum for the microorganisms that are usually found in the wounds of these patients is frequent. The development of local clinical practice guidelines is required to help reduce the overprescription of antibiotics, as the excessive use of antimicrobials is the main determinant of antimicrobial resistance.

BACKGROUND: The seizure subtype of functional neurological disorder (FND-seizures) is a common neuropsychiatric condition manifesting with episodic epilepsy-like events. Despite the common belief that FND-seizures are precipitated by psychological stressors, neurological disorders may also be triggers. In 1890, Babinski described four cases of FND symptoms associated with migraine attacks. Despite the passing of more than 130 years since this first clinical observation, the relationship between FND-seizures and migraine is not fully elucidated. OBJECTIVES: (1) To complete a systematic review of the literature that investigated potential associations between FND-seizures and migraine and the response of FND seizures to treatment with migraine prophylactic medications (2). To undertake a retrospective study of patients with FND-seizures and migraine, including response to migraine prophylaxis. METHODS: (1) Using PRISMA methods, we completed a systematic review of EMBASE and Scopus databases from inception to March 31, 2021, for literature on FND-seizures and migraine. (2) Our multi-disciplinary team, including subspecialists in psychosomatic medicine, epilepsy, and headache disorders, reviewed consecutive patients diagnosed with FND-seizures and migraine to assess potential causal associations and responses to standard migraine prophylactic medications. RESULTS: (1) The search yielded seven studies from 126 screened manuscripts (N = 1,186 patients with FND-seizures; mean age 38.7 years; 72.6% female). They varied substantially in design, population, diagnostic measures, and outcomes. Nevertheless, all studies found associations between FND-seizures and migraine, which were stronger than those between epileptic seizures and migraine in comparative investigations, but provided limited information on treatment response. (2) In our case series, investigators reached unanimous consensus that migraine attacks triggered FND-seizures in 28/43 (65.1%) patients reviewed (mean age, 38.8 years; 74% female). In 19/26 (73%) patients with adequate follow-up data, treatment with migraine prophylactic medications alone (no behavioral interventions) concomitantly reduced FND-seizure and headache frequency by >50%. CONCLUSION: Our systematic review and case series indicate that migraine attacks may trigger FND-seizures, perhaps more often that currently understood, and suggest that migraine prophylaxis may reduce FND-seizure frequency in such cases. To validate these observations, fully powered prospective investigations are required.

This study examines the prevalence and distribution of microplastic polymer types in lentic ecosystems, revealing significant heterogeneity across different geographical regions and ecosystems. The most dominant type of microplastic observed was polyethylene (PE), followed by polypropylene (PP) and polystyrene (PS), which aligns with global production rates. North America, Asia, and Europe were identified as the regions with the highest microplastic contamination, with the United States, China, Italy, and Spain being the most affected countries. The physical characteristics of each ecosystem, such as wind speed, depth, and eutrophication, alongside seasonal variations, and anthropogenic activities, contributed to the observed heterogeneity in microplastics concentrations. The study highlights the need for further research on microplastics in lentic ecosystems, considering their unique physical characteristics and anthropogenic influences. A significant lack of methodological standardization in microplastics research was identified, leading to underestimation of microplastics prevalence and high heterogeneity in meta-analyses.

Recent studies link increased ozone (O3) and carbon dioxide (CO2) levels to alteration of plant performance and plant-herbivore interactions, but their interactive effects on plant-pollinator interactions are little understood. Extra floral nectaries (EFNs) are essential organs used by some plants for stimulating defense against herbivory and for the attraction of insect pollinators, e.g., bees. The factors driving the interactions between bees and plants regarding the visitation of bees to EFNs are poorly understood, especially in the face of global change driven by greenhouse gases. Here, we experimentally tested whether elevated levels of O3 and CO2 individually and interactively alter the emission of Volatile Organic Compound (VOC) profiles in the field bean plant (Vicia faba, L., Fabaceae), EFN nectar production and EFN visitation by the European orchard bee (Osmia cornuta, Latreille, Megachilidae). Our results showed that O3 alone had significant negative effects on the blends of VOCs emitted while the treatment with elevated CO2 alone did not differ from the control. Furthermore, as with O3 alone, the mixture of O3 and CO2 also had a significant difference in the VOCs' profile. O3 exposure was also linked to reduced nectar volume and had a negative impact on EFN visitation by bees. Increased CO2 level, on the other hand, had a positive impact on bee visits. Our results add to the knowledge of the interactive effects of O3 and CO2 on plant volatiles emitted by Vicia faba and bee responses. As greenhouse gas levels continue to rise globally, it is important to take these findings into consideration to better prepare for changes in plant-insect interactions.

OBJECTIVE: We conducted an updated, comprehensive, and contemporary systematic review to examine the efficacy of existing pharmacologic agents employed for management of delirium symptoms among hospitalized adults. METHODS: Searches of PubMed, Scopus, Embase, and Cochrane Library databases from inception to May 2021 were performed to identify studies investigating efficacy of pharmacologic agents for management of delirium. RESULTS: Of 11,424 articles obtained from searches, a total of 33 articles (N = 3030 participants) of randomized or non-randomized trials, in which pharmacologic treatment was compared to active comparator, placebo, or no treatment, met all criteria and were included in this review. Medications used for management of delirium symptoms included antipsychotic medications (N = 27), alpha-2 agonists (N = 5), benzodiazepines (N = 2), antidepressants (n = 1), acetylcholinesterase inhibitors (N = 2), melatonin (N = 2), opioids (N = 1), and antiemetics (N = 2). Despite somewhat mixed findings and a relative lack of high-quality trials, it appears that antipsychotic medications (e.g., haloperidol, olanzapine, risperidone, or quetiapine) and dexmedetomidine have the potential to improve delirium outcomes. CONCLUSIONS: Pharmacologic agents can reduce delirium symptoms (e.g., agitation) in some hospitalized patients. Additional double-blinded, randomized, placebo-controlled clinical trials are critically needed to investigate the efficacy of pharmacologic agents for diverse hospitalized populations (e.g., post-surgical patients, patients at the end-of-life, or in intensive care units).

PURPOSE: To describe the demographic clinical characteristics and to identify the risk factors of patients diagnosed with fungemia and secondary intraocular involvement. METHODS: Retrospective cohort of 97 patients diagnosed with fungemia and with or without involvement of the posterior segment. Demographic, clinical and ophthalmological variables were identified to establish the risk of retinal seeding. RESULTS: An incidence of ocular involvement of 22.68% was obtained and no clear risk factor was found for subsequent showings in patients with fungemia. A risk trend was only found in patients with diabetes with an OR: 2.85; CI 95%: (0.80-10.12) and history of HIV with an OR: 2.29 CI95%: (0.85-6.12). CONCLUSIONS: In this first cohort carried out in Colombia according to our search, findings were obtained that agree with those of other authors worldwide, where there is no evidence of a decrease in incidence compared with older studies and the absence of risk factors for the compromise of the posterior pole in patients with fungemia.KEY MESSAGESSystematic fundus evaluation by an ophthalmologist in patients with candidaemia is a recommended practice based on low-quality evidence.The identification of real risk factors for retinal compromise in fungemia would allow us to be more selective with the population to be evaluated.Fungemia generally occurs in critically ill patients, where access and availability of ophthalmology evaluation are a resource that is not always available.

Exposure of plants to environmental stressors can modify their metabolism, interactions with other organisms and reproductive success. Tropospheric ozone is a source of plant stress. We investigated how an acute exposure to ozone at different times of plant development affects reproductive performance, as well as the flowering patterns and the interactions with pollinators and herbivores, of wild mustard plants. The number of open flowers was higher on plants exposed to ozone at earlier ages than on the respective controls, while plants exposed at later ages showed a tendency for decreased number of open flowers. The changes in the number of flowers provided a good explanation for the ozone-induced effects on reproductive performance and on pollinator visitation. Ozone exposure at earlier ages also led to either earlier or extended flowering periods. Moreover, ozone tended to increase herbivore abundance, with responses depending on herbivore taxa and the plant age at the time of ozone exposure. These results suggest that the effects of ozone exposure depend on the developmental stage of the plant, affecting the flowering patterns in different directions, with consequences for pollination and reproduction of annual crops and wild species.

Tropospheric ozone is a highly oxidative pollutant with the potential to alter plant metabolism. The direct effects of ozone on plant phenotype may alter interactions with other organisms, such as pollinators, and, consequently, affect plant reproductive success. In a set of greenhouse experiments, we tested whether exposure of plants to a high level of ozone affected their phenological development, their attractiveness to four different pollinators (mason bees, honeybees, hoverflies and bumblebees) and, ultimately, their reproductive success. Exposure of plants to ozone accelerated flowering, particularly on plants that were growing in autumn, when light and temperature cues, that commonly promote flowering, were weaker. Simultaneously, there was a tendency for ozone-exposed plants to disinvest in vegetative growth. Plant exposure to ozone did not substantially affect pollinator preference, but bumblebees had a tendency to visit more flowers on ozone-exposed plants, an effect that was driven by the fact that these plants tended to have more open flowers, meaning a stronger attraction signal. Honeybees spent more time per flower on ozone-exposed plants than on control plants. Acceleration of flower production and the behavioural responses of pollinators to ozone-exposed plants resulted in retained reproductive fitness of plants pollinated by bumblebees, honeybees and mason bees, despite the negative effects of ozone on plant growth. Plants that were pollinated by hoverflies had a reduction in reproductive fitness in response to ozone. In a natural setting, acceleration of flowering by ozone might foster desynchronization between plant and pollinator activities. This can have a strong impact on plants with short flowering periods and on plants that, unlike wild mustard, lack compensatory mechanisms to cope with the absence of pollinator activity in the beginning of flowering.

Abstract Introduction: Cutting is a nonsuicidal self-injurious behavior. Although this behavior is increasingly frequent in children under the age of 15, studies on its prevalence in the Colombian pediatric population have not yet been conducted. Objective: To describe the risk factors and the sociodemographic and clinical characteristics of children under the age of 15 diagnosed with cutting and treated at the emergency service of a quaternary care hospital located in Bogotá D.C., Colombia. Materials and methods: Descriptive, observational, and cross-sectional study. The medical records of patients under 15 years of age with Z91.5, F32, F33, F99, F19, F41, F43, T742 and X60 ICD-10-CM medical diagnosis codes treated between 2011 and 2017 were reviewed to identify cases meeting the diagnosis criteria for cutting (nonsuicidal self-injury). Results: In total, 85 medical records of children aged 10-14 were included. Cutting prevalence in the sample was 0.15%, but in the psychiatric disorders subgroup it was 26.7%. A 3:1 female-to-male ratio was found. Out of the 85 children, 62% did not have a nuclear family (i.e., did not live with both parents), 30.6% showed a poor school performance, and 80% injured their forearms. Concerning their mental health, 22.3% reported their anxiety was reduced after injuring themselves, and 72.9% did not have suicidal thoughts. The most frequent risk factors were having a history of mental disorder (52.9%), having experienced psychological violence (25.8%), and, in the case of boys, using psychoactive substances (22.7%). Conclusions: The prevalence of cutting found here is lower than what has been reported for this population in other countries. Likewise, cutting was three times more common in girls, and having a history of mental disorders might increase the risk of developing this behavior. Although the characteristics described here will allow identifying easily this condition in Colombian children, it is necessary to conduct further studies to determine the effectiveness of therapies aimed at this population.


Resumen Introducción. El cutting es un comportamiento de autolesión no suicida cada vez más frecuente en menores de 15 años; sin embargo, en Colombia aún no se han realizado estudios que reporten la prevalencia de esta conducta en población pediátrica. Objetivo. Describir las características sociodemográficas y clínicas, y los factores de riesgo de la población pediátrica (0 a 14 años) con diagnóstico de cutting atendida en el servicio de urgencias de una clínica de cuarto nivel en Bogotá D.C., Colombia. Materiales y métodos. Estudio observacional descriptivo de corte transversal en el que se revisaron las historias clínicas de los pacientes menores de 15 años atendidos entre los años 2011 y 2017 con los códigos diagnósticos CIE 10 Z91.5, F32, F33, F99, F19, F41, F43, T742 y X60, con el fin de identificar aquellos casos que cumplieran con los criterios diagnósticos de cutting. Resultados. Se incluyeron 85 historias clínicas de niños entre 10 y 14 años. La prevalencia de cutting fue de 0.15% en el total de la muestra y de 26.7% en el subgrupo con antecedentes de trastorno psiquiátrico. Se encontró una relación mujer a hombre de 3:1. El 62% no tenía un núcleo familiar unificado, el 30.6% presentó un rendimiento escolar bajo y el 80% se lesionó el antebrazo. Respecto a su salud mental, el 22.3% reportó reducción de ansiedad con la lesión y el 72.9% no presentó ideas suicidas. Los factores de riesgo más frecuentes fueron presentar antecedente de enfermedad mental (52.9%), ser víctima de violencia psicológica (25.8%) y, en el caso de los varones, consumir sustancias psicoactivas (22.7%). Conclusiones. La prevalencia de cutting fue inferior en comparación con lo reportado en otros países. Este comportamiento fue tres veces más frecuente en niñas y el antecedente personal psiquiátrico podría aumentar el riesgo de presentarlo. A pesar de que las características aquí descritas permitirán identificar fácilmente esta condición en población pediátrica colombiana, es necesario realizar nuevos estudios que determinen la efectividad de medidas terapéuticas dirigidas a esta población.

A 43-year-old woman underwent radiofrequency pulmonary vein ablation for symptomatic paroxysmal atrial fibrillation. At 3 months, she developed worsening dyspnea and exercise intolerance; tests revealed severe stenosis in her right pulmonary veins at the venoatrial junction and an abnormally small left atrium.

CONTEXT: Despite clear health benefits, many individuals fail to achieve the recommended levels of physical activity. Text message interventions to promote physical activity hold promise owing to the ubiquity of cell phones and the low expense of text message delivery. EVIDENCE ACQUISITION: A systematic review and meta-analysis were performed to examine the impact of text message interventions on physical activity. Searches of PubMed, PsycINFO, Scopus, Cochrane, and ClinicalTrials.gov databases from inception to December 2017 were performed to identify studies investigating one-way text message interventionss to promote physical activity. A subset of RCTs, including an objective (accelerometer-based) physical activity outcome, were included in random-effects meta-analyses in 2018. EVIDENCE SYNTHESIS: The systematic search revealed 944 articles. Of these, 59 were included in the systematic review (12 1-arm trials and 47 controlled trials; n=8,742; mean age, 42.2 years; 56.2% female). In meta-analyses of 13 studies (n=1,346), text message interventionss led to significantly greater objectively measured postintervention steps/day (Cohen's d=0.38, 95% CI=0.19, 0.58, n=10 studies). Analysis of postintervention moderate-to-vigorous physical activity found a similar but not statistically significant effect (Cohen's d=0.31, 95% CI= -0.01, 0.63, n=5 studies). Interventions with more components, tailored content, and interventions in medical populations led to nonsignificantly larger effect sizes compared with text message interventions without these features. CONCLUSIONS: Text message interventions lead to higher objectively measured postintervention physical activity compared with control groups. More extensive, well-controlled studies are needed to examine this relationship further and identify characteristics of effective text message interventions.

Abiotic stress by elevated tropospheric ozone and temperature can alter plants' metabolism, growth, and nutritional value and modify the life cycle of their herbivores. We investigated how the duration of exposure of Sinapis arvensis plants to high ozone and temperature levels affect the life cycle of the large cabbage white, Pieris brassicae. Plants were exposed to ozone-clean (control) or ozone-enriched conditions (120 ppb) for either 1 or 5 days and were afterwards kept in a greenhouse with variable temperature conditions. When given the choice, P. brassicae butterflies laid 49% fewer eggs on ozone-exposed than on control plants when the exposure lasted for 5 days, but showed no preference when exposure lasted for 1 day. The caterpillars took longer to hatch on ozone-exposed plants and at lower ambient temperatures. The ozone treatment had a positive effect on the survival of the eggs. Ozone decreased the growth of caterpillars reared at higher temperatures on plants exposed for 5 days, but not on plants exposed for 1 day. Overall, longer exposure of the plants to ozone and higher temperatures affected the life cycle of the herbivore more strongly. With global warming, the indirect impacts of ozone on herbivores are likely to become more common.

Resumen La leishmaniosis cutánea es una enfermedad causada por un parásito pro tozoo intracelular; el glucantime es una opción terapéutica, aunque está asociado con alteraciones cardiovasculares, siendo la más frecuente la prolongación del intervalo QTc que se presenta entre el 17,8 % y 19 % de los pacientes. Si este efecto no es detectado a tiempo puede causar una arritmia fatal por torsade de pointes. Se presenta el caso de una paciente de 77 años quien se encontraba en tratamiento con glucantime intramus cular como tratamiento de leishmaniosis cutánea e ingresó por un cuadro clínico de hipocalemia severa refractaria y episodios de torsade de pointes; posteriormente, presentó fibrilación ventricular que no respondió a des fibrilación y reanimación. Las alteraciones en la repolarización cardiaca producidas por este medicamento se consideran secundarias a la acu mulación de compuestos pentavalentes y trivalentes en el miocardio. No existe tratamiento específico para esta situación, pero siempre se debe realizar manejo de soporte, evitar fármacos que prolonguen el intervalo QT, normalizar los niveles de potasio y de magnesio, elevar la frecuencia cardiaca con isoproterenol e implantar marcapaso transvenoso para lograr sobre-estimulación y reducción de los periodos refractarios.


Abstract Cutaneous leishmaniasis is a disease caused by an intracellular protozoan parasite; Glucantime is a therapeutic option, although it is associated with cardiovascular alterations, the most frequent being the prolongation of the QTc interval, that occurs between 17.8% and 19% of patients. If this effect is not early recognized, it can cause a fatal arrhythmia due to torsade de pointes. The case of a 77-year-old patient who was receiving intramuscu lar glucantime as treatment for cutaneous leishmaniasis is presented, the patient was admitted with severe refractory hypokalemia and episodes of torsade de pointes; subsequently, presented ventricular fibrillation that did not respond to defibrillation and resuscitation. The alterations in cardiac repolarization produced by this medicine are considered secondary to the accumulation of pentavalent and trivalent compounds in the myocardium. There is no specific treatment for this situation, but supportive manage ment should always be performed, avoid drugs that prolong the QT inter val, normalize potassium and magnesium levels, raise the heart rate with isoproterenol and implant transvenous pacemaker to achieve over-stimulation and reduction of refractory periods.

Resumen La intususcepción intestinal consiste en la introducción o invaginación de un segmento del intestino dentro de la luz del segmento adyacente. Una parte del intestino se pliega como un telescopio y penetra dentro de otra. La ma yoría de los casos se producen durante los primeros cinco años de vida y la mitad de estos durante el primer año. En los niños mayores y en los adultos frecuentemente es el resultado de lesiones tumorales intraluminales como lipomas, carcinoma, pólipos fibroides inflamatorios o linfoma maligno. Se presenta el caso clínico de una paciente de 45 años con cuadro de dolor ab dominal originado por intususcepción ileotransversa causada por un lipoma intraluminal, el cual fue resuelto exitosamente mediante cirugía.


Abstract Intussusception involves the introduction or invagination of a segment of the intestine within the lumen of the adjacent segment. A part of the intes tine folds like a telescope and penetrates into another. Most cases occur during the first five years of life and half of these during the first year. In older children and adults are the result of intraluminal tumor lesions such as lipomas, carcinoma, inflammatory fibroid polyps or malignant lymphoma. We present the clinical case of a 45-year-old patient with abdominal pain caused by ileotransverse intussusception caused by an intraluminal lipoma, which was successfully resolved by surgery.

Fibromyalgia (FM) is a centralized pain state that until recently has been shrouded in mystery and questionable as a disease entity in the eyes of many physicians, who considered it purely psychogenic. Fibromyalgia is now thought of as a discrete diagnosis with a clustering of symptoms characterized by central nervous system pain amplification along with anergia, memory loss, disturbances of mood, and sleep disruption. The condition is present in approximately 2% to 8% of the population. We review the link between inflammatory mechanisms and FM from a neuropsychiatric perspective. Recent studies are pointing to a neuroinflammatory etiology that may open up more effective treatment strategies in the future. Better conceptualization of FM may also elucidate a neuropsychiatric understanding of how nociception, dysthymia, and suicidality co-develop and feed off one another.

OBJECTIVE: Adherence to health behaviors following an acute coronary syndrome (ACS) is strongly associated with superior prognosis. Both optimism and positive affect may play important roles in such adherence. However, the relationship between changes in these constructs in the context of positive psychology interventions (PPIs) and adherence to health behaviors is not fully understood. Accordingly, we aimed to examine these relationships among a cohort of post-ACS patients receiving a PPI. METHOD: Post-ACS participants who received a PPI during a factorial trial (N = 128) completed self-report measures of positive affect and optimism, along with the Medical Outcomes Study Specific Adherence Scale items for diet, physical activity, and medication adherence, over 16 weeks. The baseline and longitudinal effects of positive affect and optimism-representing changes in those constructs-on adherence were analyzed using mixed effects regression models. RESULTS: Positive affect, but not optimism, was longitudinally associated with greater overall adherence to health behaviors (positive affect: ß = 0.057, p = .006; optimism: ß = 0.032, p = .36), with the effect driven by physical activity adherence (positive affect: ß = 0.040, p = .004; optimism: ß = 0.005, p = .83). CONCLUSION: Changes in positive affect may be more strongly associated with post-ACS adherence than optimism; this could have important implications for the development of PPIs to promote adherence.

Resumen Introducción: el bostezo se presenta con frecuencia en los pacientes sometidos a prueba de mesa basculante. Estudios previos han demostrado que el bostezo estimula el sistema nervioso simpático con sus consecuentes cambios hemodinámicos, principalmente en la frecuencia cardíaca y la presión arterial. Objetivo: describir los efectos hemodinámicos y sobre el sistema nervioso autónomo del bostezo, así como establecer el bostezo como mecanismo protector de síncope o predictor de respuesta hemodinámica positiva. Métodos: estudio observacional de fuente secundaria, realizado a partir de una base de datos de pacientes que fueron sometidos a prueba de mesa basculante entre agosto de 2010 hasta diciembre de 2015 y presentaron al menos un bostezo durante la prueba. Se realizaron análisis estadísticos y pruebas pareadas para determinar correlaciones. Resultados: se incluyeron 156 pacientes, la mayoría mujeres, edad promedio 40 años. 43 pacientes presentaron síncope, 113 (72,4%) bostezo sin síncope, y de éstos, 50 (44,2%) tuvieron bostezo y prueba de mesa basculante con respuesta positiva. Se observó un aumento en la mayoría de variables hemodinámicas, sin embargo, hubo una disminución significativa de la frecuencia cardíaca. En 67 pacientes se observó bostezo como protector de síncope mientras que en 61 pacientes como predictor de respuesta hemodinámica positiva. Conclusión: el bostezo se asoció con activación del sistema nervioso simpático, con efectos hemodinámicos y sobre el sistema nervioso autónomo. Puede plantearse como un signo predictor de respuesta hemodinámica positiva de tipo mixto o vasodepresora y en otros casos como un mecanismo protector frente a la aparición de síncope.


Abstract Introduction: Yawning is often present in patients subjected to a tilt table test. Previous studies have demonstrated that yawning stimulates the sympathetic nervous system, with its subsequent haemodynamic changes, mainly heart rate and blood pressure. Objective: To describe the haemodynamic effects of yawning on the autonomic nervous system, as well as to establish yawning as a protective mechanism of syncope, or a predictor of a positive haemodynamic response. Methods: A secondary source observational study conducted using a database of patients that were subjected to the tilt table test between August 2010 and December 2015, and yawned at least once during the test. Statistical analyses and paired tests were performed to determine correlations. Results: A total of 156 patients were included, the majority women, and a mean age of 40 years. Syncope was presented in 43 patients. The remaining 113 (72.4%) yawned with no syncope, and of these, 50 (44.2%) had yawned and had a positive response to the tilt-table test. Although there was an increase in the majority of haemodynamic variables, a significant decrease was observed in the heart rate. Yawning was observed as a protector of syncope in 67 patients, whilst it was a predictor of a positive haemodynamic response in 61 patients. Conclusion: Yawning is associated with activation of the sympathetic nervous system, with haemodynamic effects and on the autonomic nervous system. It could be established as a predictive sign of a mixed type or vasodepressor positive haemodynamic response, and in other cases as a protective mechanism against the appearance of a syncope.
